This Transition Plan Survey resource helps you gather information from caregivers and students alike for the student’s individualized transition plan (ITP).
You can use this every year before the ITP/IEP meeting to see where the student is and what they want to work on next. You can also use this at the start of the school year or with new students to get up to speed on what transition activities they have completed and are interested in.
This survey resource is comprehensive and covers all of the critical components of the transition plan: postsecondary education, employment, and independent living.
ACROSS AGES AND SUPPORT NEEDS
Our resources are made with images and icons that support learners across high school years and into adult day programs. Students of any age can use this particular resource while still finding it age-respectful. Use with students across a wide variety of support needs. We’ve included visuals and multiple-choice options to increase student engagement and responses for those who need it, as well as, spaces for students who write more.
IDEAS FOR USE
Before annual meetings, send before IEP pre-draft to caregivers, welcome packets, open house nights, back to school, start of the school year, first week of school, last week of school to go along with the student to their next teacher/school, student-led IEP preparation, study skills/resource time
PROMOTING NEURODIVERSITY + FUTURE-FOCUSED WORK
This survey is completely student-centered and frames questions around common self-advocacy and transition-related needs students have as they approach adulthood years (e.g., getting to know their needs and goals, figuring out what employment activities they would like to do, and tracking progress towards their goals).
